PRE-REGISTRATION REQUIREMENTS
All student-athletes must have completed the following items BEFORE they can try out, practice or participate. Please ensure that all of your information is completed and uploaded to Magnus Health Student Health Tracker (magnushealthportal.com). This includes a current Physical Exam Form (dated within one calendar year). The deadline to submit this information is October 31, 2025.
In addition, view the SFHS Athletic video and complete the online forms listed below, via our online athletic registration, Snap Manage. Please log in to

NOTE: These forms must be completed online with an electronic signature. All online forms are due no later than October 31, 2025. Participation fees for no-cut sports are paid online at the time of registration. Participation fees for cut sports are paid after rosters are confirmed.
- A signed Parent Permission form for athletic participation
- A signed IHSA Sports Medicine consent form
- A signed Co-Curricular Participation contract
Winter coaches will receive a list of participants who have completed all forms and are cleared to participate. Please check with your coach 30 minutes before the start of the first try-out/practice session to ensure that your name is on the list. If you are not, then you have not been cleared to participate/try-out. If this is the case, please report to the Athletic Director’s office immediately to determine what is needed to be cleared for participation. Once you are cleared, you will be given a signed clearance pass to hand to your coach so you can begin participation. You will not be allowed to participate without being on the list or receiving clearance from the Athletic Office. Sorry, no exceptions.
