Fall Sports Tryout Information
PRE-REGISTRATION REQUIREMENTS
All student-athletes must have completed the following items BEFORE they can try out, practice or participate. Please ensure all your information is completed and uploaded on Magnus Health. This includes a current (within one calendar year) Annual Physical Exam Form. The deadline to submit this information is Monday, July 21, 2025.
In addition, complete the following forms online, via our athletic online registration, Snap Manage. Please logon to
Snap Registration. NOTE: These forms and video
must be completed online with an electronic signature. All online forms are due no later than
Wednesday, July 30, 2025.
- A signed Parent Permission form for athletic participation
2. A signed IHSA Sports Medicine consent form
3. A signed Co-Curricular Participation contract
4. A signed Student-Athlete Consent to Treat form
Registration fees are paid online after rosters are confirmed for cut sports. For no-cut sports, registration fees are paid online at the time of registration.
Fall coaches will receive a list of participants who have completed all forms and are cleared to participate. Please check with your coach 30 minutes before the start of the first try out/practice session to ensure that your name is on the list. If you are not, then you have not been cleared to participate/try out. If this is the case, please report to the Athletic Director’s office immediately to determine what is needed to be cleared for participation. Once you are cleared, you will be given a signed clearance pass to hand to your coach so you can begin participation. You will not be allowed to participate without being on the list or receiving clearance from the Athletic office. Sorry, no exceptions.
