Illinois Youth Survey Information

On Thursday, April 23, sophomores and seniors are scheduled to take the Illinois Youth Survey (IYS). This survey, administered statewide, gathers information on youth attitudes and behaviors that can impact student problems and academic success. The survey asks about topics including nutrition, physical activity, bullying, tobacco, alcohol, and other drug use. The survey is confidential and privacy is strictly protected - no names or identification numbers are used. The test is administered to sophomores during Health/P.E. and to seniors during Religion.
This survey is optional. If you do not want your child to participate, please return this form to Mrs. Rupp in the Counseling Department before Tuesday, April 9. On the day of the survey, students are also given the choice to participate or not. Students who do not participate are provided an alternative, neutral activity (quiet reading) in the classroom.
 
How is the survey data used? The IYS provides SFHS with a report of combined student results to help determine which programs may be needed to keep students safe and healthy.
